I cannot find my invitation.Check spam/junk and institutional quarantine. Ask the study team to confirm the address and resend your own link.
The link says the survey is closed.Contact the study team about the participation period.
Access is denied.Use the full original link and ask the study team to check your group membership and current access.
A response limit has been reached.The administrator needs to review the survey or account allowance.
I am prompted to register again.Check that you are using your personal invitation, rather than a shared registration link.
My answers have not appeared on another device.Save on the original device first and use the same personal link.
Next will not work.Read the validation message; check required answers, comments, and matrix inputs.
My progress is not 100%.Review To Do and required items, then ask the study team if the remaining items are optional or skipped.
Feedback is missing.Save your answer and review the study’s feedback instructions; feedback may be delayed or disabled.

Protect your questionnaire

Your personal invitation is an access link to your answers. Do not share it with another participant or post it publicly. If someone else has used it or the displayed answers are unexpected, contact the study team promptly.

Before refreshing, closing the page, clearing browser data, or trying another browser, save if possible. Unsaved edits and anonymous sessions may be lost.

Ask for help clearly

Tell the study team what happened, the page or question title, your browser/device, and the exact error message. A screenshot can help, but remove private links and personal information before sharing it outside the study team.

For withdrawal, mail opt-out, or data deletion requests, follow the study information sheet or contact the study team. These are different actions and can have different effects on your access and recorded answers.
